MRI Safety Teleconference - June 25, 2008

The speaker, Emanuel Kanal, MD, FACR, AANG is a Professor of Radiology and Neuroradiology at the University of Pittsburgh Medical Center and is the Director of Magnetic Resonance (MR) Services at the University of Pittsburgh. In 1991, he was the first to have developed and presented the usage of timed bolus contract enhanced magnetic resonance angiography in humans.

 

The American College of Radiology's MR Safety Committee has again updated their Guidance Document for Safe MR Practices in late 2007. This newly revised has been revisited and completely updated, with some section completely rewritten and several new ones added for the first time. The Guidance Document represents the unanimous consensus of all members of the ACR MR Safety Committee and is now more comprehensive than ever before. Topics covered address the ongoing medical MR safety education of the health care practitioners and how they are to behave in the MR environment, the site preparation, device and implant safety testing, device labeling, site preparation and siting considerations, patient screening practices, etc. A special rather comprehensive section was added to address the relatively recent safety considerations regarding gadolinium-based MR contrast agents, renal disease patients, and the potential to develop nephrogenic systemic fibrosis. Taken collectively, these are designed to assist MR practitioners in defining their own site's MR Safety Policies and Guidelines to help create a more safe environment for the patients and health care professionals alike. 2008 High Stakes Communication Series

Communication has been identified as a primary issue in Root Cause Analysis during Sentinel Event investigation. Communication is a critical aspect of ensuring the health and safety of patients. Because of this, OPSI has chosen to focus on High Stakes Communication for its 2008 conference series in order to improve patient safety processes. - REGISTER NOW!

 

Each session will be one hour in length beginning at 11:30 a.m. Eastern Time.

         

          • May 14 – Hand Off Communication

                Sharon Walsh-Hart will discuss hand off communication as it relates to the Joint Commission   

                Patient Safety Goals. She will identify various strategies for hand off communication and how to      

                apply them to your current practices. Strategies include SEAM approach, SHARED methodology, I

                PASS the BATON, Team STEPPS, 5 P's, and Electronic Technology.

Anticoagulation Therapy Collaborative - May 15, 2008

This program will help organizations initiate or continue their planning and implementation of strategies to assure safe use of high risk medications as well as the attainment of National Patient Safety Goal 3E.  Participants will:
  • understand from the patient’s perspective the compelling case for safe use of anticoagulants,
  • describe the safety risks associated with use of these agents,
  • learn strategies for assuring safe processes, and
  • implement a data collection process that will allow them to track their progress over time.
For the 6 month project the cost for non-VHA hospitals is $600. This includes the following deliverables:
  • One day educational session on May 15, 2008 (4 participants)

  • Data tool to report compliance with heparin and warfarin protocols (where implemented)

  • Data tool to track dynamics of lab values (INR's, a PTT's) longitudinally

  • Self-assessment tool outlining key strategies recommended for safety improvement

  • Three group conference calls

  • One individual coahcing call

  • Resource material

Enrollment
Hospitals enrolling in the collaborative must complete the registration form, participation and data release (Appendix A) and the Team List (Appendix B)  by April 10, 2008
and return to Julie Barbieri by fax at (317) 818-8008. 

 

Teams enrolling between April 10th-25th may still have the opportunity to complete and receive feedback on the self-assessment tool prior to the May 15 conference, but this is not guaranteed. Teams enrolling after April 25 will receive feedback on their self-assessment tool after the conference. Enrollments will be processed on a first-come, first-served basis, so early registration is encouraged.
