Submit an Effective Practice
If you would like to submit a patient safety effective practice that was implemented by your organization, please send OPSI the following materials.  For publication on the website, a release form is required.

• One-sentence description for website navigation
• Abstract: a brief summary of the effective practice that was implemented, including information validating the effectiveness of the practice
• Submission form
• Release form
• Optional Supportive Information: A comprehensive review, not to exceed 10 pages, that includes, but is not limited to:
  • Description of Organization
  • Rationale for implementation of specific practice
  • Policies and procedures involved in implementation
  • Data collection instruments used, if applicable
  • Data results achieved, as available
  • Analysis of data, as available
  • Evaluation of outcomes, as applicable
  • Validation of effectiveness
  • Practice assessment tool, as available
  • Algorithms and pathways involved
  • References
  • Forms created
  • Power Point presentations created
  • Staff education materials
