Kathy Crea, PharmD

Kathy Crea is a pharmacist by training and has extensive experience in both patient and medication safety. She has worked at both a hospital and system level, coordinating a system wide adverse drug event trigger project that was the recipient of the first Award for Excellence in Medication Use Safety, along with a Best Practice Award from ASHP. She is a member of the Ohio team in the National Patient Safety Corps (2004 class) sponsored by AHRQ and the VA; is trained and teaches Root Cause Analysis, is trained in TeamStepps (team training), and has presented many posters and presentations at various local and national meetings. She is also Associate Clinical Professor at the University of Findlay School of Pharmacy, where she is responsible for incorporating patient and medication safety principles into the curriculum.
